Why is this study being done? 

We want to know about attitudes about certain health behaviors and health policies in Oklahoma.  This survey covers sensitive topics about tobacco, nicotine, and vaping behavior. Whether or not you use any of these, it’s important that you answer honestly so that this research reflects reality.  

What will I be asked to do? 

If you decide to join the study, you will be asked to answer questions about how you feel about tobacco, nicotine, and vaping and if you have or have not used these before. You do not have to answer any question that makes you feel uncomfortable, and there is no right or wrong answer to the questions. This screening questionnaire will only take about 2-3 minutes to complete, and the full research study will be conducted over a 3-day period, from [date] to [date]. You can expect to spend about 30-minutes per day during the study. 

Will anyone know how I answered the questions? 

Your answers will not be connected with your name or anything that identifies you. Your parents or guardians will not be told how you answered the questions or if you answered the questions. 

Are there any bad things that can happen to me by being in the study? 

You may feel uncomfortable answering some of the questions. You do not have to answer any question that makes you feel uncomfortable.  You will not get in any legal trouble for any of your answers.  

Are there any good things that can happen to me by being in the study? 

You may be more aware of your behaviors and attitudes, and you’ll experience the satisfaction of influencing an initiative to help teens across Oklahoma lead healthier lives. However, there is no guarantee of any particular benefit to participating in the study. You will also receive $175 via Venmo, Cash App, or check for participating in all 3 days of the study.  

Can I refuse to take part in the study? 

Your parents or guardians have said that if you want to be in this study, it is okay with them. You don’t have to be in the study if you don’t want to. You can stop being in the study at any time. You will receive partial compensation if you start the study but do not participate in all 3 days. 

This research study will be conducted online from November 4 – 6 and you can expect to spend about 30-minutes per-day answering the questions. The format is similar to an anonymous online forum, where each day a series of questions will be posted and you will type in your responses. Once you answer a question, the other participants’ responses will be revealed, and you can comment on what they said.  

To thank you for your time, you will receive $175 via your choice of Venmo, Cash App, or check for participating in all three days of the study.
