Thank you for agreeing to participate in our upcoming research study. The details for the session that you have been invited to are:
*Free parking for the research project is available on Boulder Avenue between the Tulsa Area United Way and Fitzgerald Funeral Service, Ivy Chapel. Jurors will need to check in at the security desk upon arrival and should have their license plate number available for building check-in. Below is a map with the designated parking area outlined in yellow for reference.
Street parking is also available; however, it is paid parking and can be paid through the ParkMobile app or at the nearby meters. Please note that the City of Tulsa does ticket vehicles without payment. If you choose to park here, you will be responsible for the cost.
You must complete the Confidentiality Agreement (password 082026) and Intake Form (password 82026) prior to the session day. If these are not completed before the day of the session you may not be allowed to participate.
Please plan to arrive and check-in at least 15 minutes early.
You will receive at the end of the session.
Please bring your picture ID with you to the facility. If you use reading glasses/contacts or hearing aids, please bring them as well. You may wish to bring a light jacket as the room temperature may vary.
Because of limited space, we cannot allow others to be admitted into the focus group. Only those who we have contacted will be accepted into this session. Childcare cannot be provided.
We have invited a limited number of people and we are counting on your timely attendance and participation for completion of our study. Your opinions are very important to us. If for any reason you cannot make this appointment, please respond upon receipt of this letter so that we may find a replacement for you.
All of our groups are conducted strictly for research purposes. At no time will anyone attempt to sell you any product or service nor will your name be supplied to any manufacturer for sales purposes. Finally, it is likely that the group will be audio and video taped for record keeping and reviewing purposes.
